Consent for Dental Services

Online Consent for Dental Services (Secondary School Dental Programme)

To promote good oral health for your child / ward, the Health Promotion Board (HPB) continues to provide free dental checks and basic treatments (if required) at specific times during school hours. The dental service for our students is provided at Fairfield Methodist School (Primary).  

To give consent:
1. Log on to https://consent.hpb.gov.sg 
2. Login using your SingPass with 2-factor authentication (2FA).
3. Type in your child’s/ward’s UIN/BC/FIN number.
4. Fill up and submit the consent form.
