Online Consent for HPV Vaccination for Secondary 1 Girls

The Health Promotion Board will be offering school-based human papillomavirus (HPV) vaccination to all female students in secondary schools, under the national school-based HPV vaccination programme.  

We will inform all parents once we have confirmed the vaccination date for Year 2025. Parents / Guardians will need to give their consent before the child / ward can receive the vaccination.  

• Students who are 14 years and below in Secondary 1 will require 2 doses. The first dose will be given in Secondary 1 and the second dose will be given in the following year. 

• Students who are 15 years old and above in Secondary 1 will require 3 doses. The first and second doses will be given in Secondary 1 and the third dose will be given in the following year.  

To give consent: 

1. Log on to https://consent.hpb.gov.sg 

2. Login using your SingPass with 2-factor authentication (2FA). 

3. Type in your child’s/ward’s UIN/BC/FIN number. 

4. Fill up and submit the consent form.
